B肝疫苗 未必一勞永逸！ 研究發現 15年後1成喪失免疫力 【謝鎔鮮】注射B肝疫苗也未必一勞永逸。學者研究發現，疫苗接種15年後可能失去免疫力，即使孩子一出生就注射，到15歲時仍有近1成的人B肝免疫力減退，甚至失去B肝免疫反應，感染風險升高。 台灣慈濟大學原住民健康研究所副教授王豊裕與慈濟醫院台北分院肝膽腸胃科醫師林憲宏，日前發表「環境、族群與B型肝炎疫苗反應性」研究成果。 王豊裕指出，台灣於1986年後全面推行新生兒注射B肝疫苗，孩子1歲前須施打3劑疫苗，周歲後可免費追加1劑。他與林憲宏以花蓮兩所高中新生健康檢查資料為基礎分析，發現約有7%至11%的高一生對HBsAg（B型肝炎病毒表面抗原）喪失免疫反應。 也就是說，即使孩子一出生就施打完整B肝疫苗，到15歲時，有近1成孩子的免疫力減退，甚至不再對B型肝炎免疫。 研究指出，1986年7月後出生的學生，顯示體內有B肝病毒存在的HBsAg陽性反應率為1.9%，明顯低於7月以前出生者的9.3%，估計B肝疫苗成功預防78%的B肝慢性感染，計畫成效良好；但即使補注疫苗，如果在一個月內有吸菸、飲酒或嚼檳榔行為者或原住民，疫苗效果會較差。 王豊裕說，B肝疫苗有效但非萬能，有些孩子施打疫苗後仍感染B肝，原因之一可能是出生時即經母親產道垂直感染，等到出生3、5天才注射疫苗，防治功效已慢一步。 2008-10-15 Liver B vaccine may not be settled once and for all!
