補充鈣,你吃對了嗎?

【陳惠惠】如果你已做過骨質密度檢測,確定自己是骨質疏鬆患者,醫生可能開給你什麼藥?

猛吞維骨力?狂吃軟骨素?或任何跟「骨」字扯得上邊的產品,都拿來「顧筋骨」?醫師說,骨頭要好,關鍵就是30歲前存足骨本,一旦骨質疏鬆,除了吃藥治療,還要補充鈣片。

骨鬆兩種藥 破骨與造骨

骨質疏鬆症的用藥治療,近年已有長足的進步,現階段用來治療骨質疏鬆的藥物可分成兩大類,以往的藥物主要為「破骨抑制劑」,最近則有「造骨促進劑」供民眾選擇。

台灣馬偕醫院骨科主治醫師張定國說,目前較常用的是破骨抑制劑,至於造骨促進劑雖可快速提升骨質密度,但價格相對昂貴。

除了吃藥,骨鬆患者還要吃鈣片、飲食多攝取含鈣食物,「鈣質才是骨頭主要成分。」

台北振興醫院骨科主治醫師曾國峰說,沒有原料,不管怎麼調控造骨或破骨細胞都沒用。

骨鬆補鈣片 天然未必好

市面上鈣片成分頗多,像是碳酸鈣、磷酸鈣、檸檬酸鈣、乳酸鈣跟葡萄酸鈣等,甚至還有珊瑚鈣、牡蠣鈣等,民眾往往不知何選擇。「不見得天然的最好。」曾國峰說,標榜萃取自生物的鈣,很可能有重金屬汙染的疑慮,化學合成鈣的品質反而較穩定。

元素鈣含量 吸收的關鍵

「一般成年人只要飲食均衡,不須特別補充鈣片。」張定國說,人體對鈣片的吸收率只有一到兩成,適當的維他命D可以促進鈣的吸收,選購時,可考慮額外添加維他命D的鈣片。

另外,據曾國峰觀察,很多民眾只見包裝上標示,每顆碳酸鈣含量1000毫克,以為只要吞1顆,就可以攝取1000毫克的鈣,那可就錯了。

「選購時,不光是看總劑量,還要注意元素鈣含量。」曾國峰說,很多人只知道吃鈣片,卻不曉得吃進多少鈣,鈣片裡,可以被吸收利用的就是「元素鈣」。

例如,1000毫克碳酸鈣裡,元素鈣的含量通常只有400毫克。

體質大不同 好壞無絕對

骨科醫師眼裡,鈣片沒有絕對好壞。曾國峰舉例,像檸檬酸鈣、乳酸鈣跟葡萄酸鈣的元素鈣比率雖相對低,但吸收度較好,葡萄酸鈣則較少用。

鈣片分為藥品級及食品級,若醫師建議使用藥品級鈣片,可參考藥品使用說明書,上面有寫明元素鈣含量。

他建議,除了看元素鈣含量高低,還要視個人身體狀況,有些鈣片元素鈣含量雖高,但下肚後卻易便秘、脹氣,還是要找出最適合自己的鈣片。
